The Department of Family and Community Medicine is seeking a Nurse Practitioner or Physician Assistant to join the  Primary Care APP – Team-Based Care Model to serve as an embedded advanced practice provider within the UC Health Burnet Practice. This role is designed to expand access, improve quality of care and reduce burden across physician practices by delivering high-quality, team-based patient care. Rather than maintaining an independent patient panel, the NP provides flexible clinical coverage across a range of visit types in close collaboration with primary care physicians. The position is situated within an academic medical environment and carries a meaningful educational component, including direct engagement with learners from medicine, nursing, pharmacy, and other health professions. The APP reports primarily to the Medical Director at UC Health Burnet, with secondary oversight from the Department Chair.

Essential Functions

Acute Care Coverage

  • Evaluate and manage acute, episodic illness and injury for established patients across physician panels.

Inbox and Messaging Management

  • Review and respond to patient messages, prescription refill requests, lab and imaging results, and care team communications within established turnaround standards.
  • Coordinate follow-up actions arising from inbox tasks, including ordering additional diagnostics, adjusting medications, or scheduling appointments.
  • Exercise sound clinical judgment to identify messages requiring urgent physician review and route them appropriately.

Annual Wellness Visits

  • Conduct Medicare Annual Wellness Visits (AWV) and other preventive health visits .

Chronic Disease Follow-Up

  • Provide scheduled follow-up visits for patients with common chronic conditions including, but not limited to, hypertension, type 2 diabetes, hyperlipidemia, obesity, and asthma/COPD.
  • Collaborate with care coordinators, clinical pharmacists, behavioral health staff, and other team members to support comprehensive chronic disease management.

Teaching and Education

  • Serve as a clinical educator and role model for interdisciplinary learners including medical students, residents, advanced practice students (NP/PA), nursing students, pharmacy students, and other health professions trainees rotating through the department.
  • Model interprofessional collaboration, professionalism, and evidence-based clinical reasoning in all interactions with learners and care team members.

Collaboration and Communication

  • Function as an integrated member of the physician practice team, attending relevant team huddles and department meetings as scheduled.
  • Maintain open and proactive communication with physicians regarding shared patients, clinical questions, and care transitions.
  • Participate in quality improvement initiatives, departmental projects, and continuing education activities as directed by the Medical Director or Department Chair.

Minimum Requirements

Nurse Practitioner

Required Education

  • Master’s Degree in Nursing from an accredited nursing program.

 

Required Trainings/Certifications

  • Active Ohio APRN license with prescriptive authority.
  • Active Board Certification in appropriate nursing specialty.
  • Active DEA license.

 

Required Experience

  • Three (3) years nursing experience.
  • Additional related work experience may be required depending on specialty/sub-specialty.

Physician Assistant

Required Education

  • Master's Degree from an accredited Physician Assistant program.

 

Required Trainings/Certifications

  • Active Ohio Physician Assistant license with prescriptive authority.
  • Certification as a Physician Assistant by the National Commission on Certification of Physician Assistants.
  • Active DEA license.

 

Required Experience

  • Additional related work experience may be required depending on specialty/sub-specialty. 

Additional Qualifications Considered

  • Strong clinical assessment and diagnostic reasoning skills across a broad range of primary care presentations.
  • Proficiency with electronic health record (EHR) systems; experience with inbox management workflows is a plus.
  • Excellent written and verbal communication skills, with the ability to work effectively within a team-based care model.
  • Ability to manage a high-volume, varied schedule while maintaining quality and patient safety standards.
  • Commitment to evidence-based practice, continuous learning, and departmental quality goals.
