The Department of Family and Community Medicine is recruiting an Advanced Practice Provider to join the actively evolving cancer cognitive and survivorship program within the UCHealth system. The provider will offer outpatient services primarily at the Cancer Cognitive Clinic and Barrett. They will provide care in collaboration with the interdisciplinary team and further the COM mission through excellence in education, innovation and clinical care in the field of cancer survivorship.

Essential Functions

  • Clinical Care- Collaborate with the interdisciplinary team to provide cancer cognitive care and general survivorship care for the patients at UC Health; Collaborate with the attending physician on clinical cases and/or seek supervision/joint visits when felt necessary by either the APP or attending physician; Attend monthly cancer wellness clinic meetings; Participate in quality improvement initiatives within the clinical sites.
  • Teaching- Contribute to a positive learning environment for learners; Participate in the journal club; Provide education and serve as a resource for patients in area of clinical expertise.
  • Administrative- Clinical Administrative Duties that include review of test results, assistance with patient phone calls and messages, and overall coordination of care in collaboration with nursing and attending physicians.
  • Expansion of general cancer cognitive clinic to Drake Hospital.
  • Creation of educational opportunities for both patients and staff

Minimum Requirements

  • Certification as a Nurse Practitioner or Physician Assistant by the appropriate boards.
  • Ability to obtain an unrestricted Ohio Nurse Practitioner or Physician Assistant License and DEA license.
  • Minimum of 3 years of clinical experience as an APP or 3+ years of any clinical experience in hospice and/or palliative medicine or a related field.
  • Communication skills to lead family meetings, goals of care discussions, and end-of-life discussions with cultural humility.
  • Ability to work within an interdisciplinary team and provide consultative care in collaboration with primary medical teams.
  • Willingness to participate in additional training in the field of Hospice and Palliative Medicine.

Additional Qualifications Considered

Prior experience in hospice and/or palliative medicine.  In the absence of hospice or palliative medicine experience, experience caring for patients in the intensive care unit, oncology, cardiology, or geriatrics setting.
